Physical Exam Forms
All students who plan to participate in NSAA sanctioned athletics during the 2009-2010 school year must have a physical exam. This exam must be completed by the physician of your choice and the clearance form must be returned to the school prior to the first practice of the sport. The consent form must also be completed and returned to the school.
